Recurring Credit Card

We only accept Discover, MasterCard and Visa.

To set up automatic Credit Card donation:

  • Please complete Recurring Credit Card Authorization, sign, and return.
  • These forms can be mailed to PO Box 460, Apple Creek, OH 44606, emailed to interlinkfamily@aol.com. Once received we will set up your information monthly support.

Please note:

  • Recurring Credit Card donations run on or about the 15th of each month.
  • You will not receive a receipt for recurring donations, unless you fill out the portion of the form that requests a monthly receipt.
  • You will receive a statement, including all donations of the previous year, in January for tax purposes.
  • If you wish to change your information or cancel your support, please email us any changes or cancellation request. This information cannot be taken over the phone as we need written confirmation of these changes/requests.
